Get Better Together is a chronic disease self management program that is designed to help those that live with chronic health problems improve their life quality on a daily basis. Chronic health problems refer to a health issue that stays with you and cannot be cured, you may feel good one day but you never know what the next day will bring ... The program is 2 ½ hours per week and runs for 6 weeks. Throughout the 6 weeks, leaders and participant will discuss various topics such as: Healthy eating, physical activity, breathing techniques, relaxation, medication, pain management and more. Each discussion leads to a new tool that can be applied to daily life and help improve the symptoms related to the long –term health problem. At the end of the sessions, each participant goes home empowered to take control over their health problem.
The program is for adults of all ages and is not specific to one chronic condition but applies to all of them, as they share multiple symptoms. If you live with a chronic condition such as; high blood pressure, heart disease, cancer, diabetes, arthritis, fibromyalgia, anxiety, depression, multiple sclerosis, chronic pain or other and want to take action to live better register now!!...
